Admissions

You must bring the following information with you to register your child.

1.Your child’s birth certificate

2. Your child’s baptismal certificate

3. Your child’s immunization records

4. Your child’s latest report card and standardized test scores (grades 1-7)

5. Two completed Recommendation Forms from your child’s current teacher and principal (grades 1-7)

6. Your child’s Social Security Card

7. Proof of custody (if other than the natural parent)

8. Names and numbers of three emergency contact people

9. If Applying for Tuition Assistance: verification of income required (professionally prepared 1040 Tax Return, Social Service Grant Letter, Disability Grant Letter, etc.) A personal interview is also required for all tuition assistance applications.

New student and parent orientation.

All new students entering grades 1-8 must take the St. Ambrose Catholic School Placement Tests in reading, writing, and math. The school reserves the right to place students in the appropriate grade level based on the results of this test. Students entering pre-kindergarten and kindergarten are tested by the Title I Program at the beginning of the school year. Parents and guardians of all new students are required to attend an orientation session with the principal and the pastor prior to the opening of school.
